By using the Site, you agree to the collection and use of information in accordance with this policy. If you do not agree with the terms of this policy, please do not use the Site.
1. Information We Collect

1.2 Technical and Automatically Collected Information
When you submit a form on the Site, Gravity Forms automatically records certain technical information along with your submission, including:
- Your IP address
- Date and time of submission
- Browser type and user agent
- Referring URL
We also may collect similar technical information (such as IP address, browser type, device type, and pages visited) through standard web server logs and analytics tools, even if you do not submit a form.
1.3 Cookies and Tracking Technologies
The Site may use cookies or similar tracking technologies to support functionality (such as spam protection on forms) and to understand how visitors use the Site (via analytics tools, if enabled). You can control or disable cookies through your browser settings; doing so may affect certain Site features.
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.
If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.
When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.
If you edit or publish an article, an additional cookie will be saved in your browser. This cookie includes no personal data and simply indicates the post ID of the article you just edited. It expires after 1 day.
2. How We Use Your Information
We use the information collected through forms and automatically through the Site to:
- Respond to inquiries and appointment requests
- Schedule and manage patient care coordination
- Communicate with you about our services
- Improve, secure, and maintain the Site
- Detect and prevent spam, fraud, or abuse of our forms (IP addresses are used for this purpose)
- Comply with legal, regulatory, or record-keeping obligations
We do not sell your personal information.
3. Protected Health Information (PHI) and HIPAA
Salem Pediatric Clinic is a healthcare provider and, in the course of patient care, may create, receive, or maintain Protected Health Information (“PHI”) as defined under the Health Insurance Portability and Accountability Act (HIPAA).
This website privacy policy addresses information collected through the Site itself (such as through contact or intake forms). It does not replace or modify our HIPAA Notice of Privacy Practices, which governs how we use and disclose PHI collected in connection with treatment, payment, and healthcare operations.
Please do not submit sensitive medical details through general contact forms unless the form is specifically designated and secured for that purpose. If a form on the Site is used to collect patient health information, that information is handled in accordance with our HIPAA Notice of Privacy Practices, available at https://salempediatricclinic.com/notice-of-privacy-practices/ OR upon request at our office.
If you have questions about how your child’s medical records are used, please contact our Administrator, 503-362-2481.
4. Children’s Privacy (COPPA)
Because Salem Pediatric Clinic serves pediatric patients, forms on this Site may request information about a minor child, submitted by a parent or legal guardian. This Site is not directed at children for the purpose of independently collecting information directly from them, and we do not knowingly collect personal information directly from children under 13 without parental/guardian involvement, consistent with the Children’s Online Privacy Protection Act (COPPA).
Any information about a minor submitted through the Site is provided by, and with the consent of, the child’s parent or legal guardian for the purpose of scheduling care or communicating with our practice.
5. Third-Party Service Providers
We use third-party vendors to help operate the Site and process form submissions, including:
- Gravity Forms (form processing and submission handling)
- DreamHost (Web Hosting Provider)
- Google Analytics
- Intellichart.com (Patient portal)
These providers may process personal information (including IP addresses) solely to provide their services to us and are expected to safeguard that information appropriately. We do not authorize these providers to use your information for their own independent marketing purposes.
6. Data Security
We use reasonable administrative, technical, and physical safeguards designed to protect the information submitted through the Site from unauthorized access, use, or disclosure. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.
7. Data Retention
We retain information submitted through Site forms for as long as necessary to fulfill the purposes described in this policy, respond to your inquiry, or comply with legal, medical record-keeping, or regulatory requirements. Patient medical records are retained in accordance with applicable state law and our HIPAA policies.
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
8. Your Choices and Rights
Depending on your location, you may have rights regarding your personal information, which may include the right to:
- Request access to the information we hold about you
- Request correction of inaccurate information
- Request deletion of certain information (subject to our legal and medical record-keeping obligations)
- Opt out of non-essential communications (e.g., newsletters)
To exercise these rights, contact us using the information in Section 11 below. Please note that requests related to medical records are handled under our HIPAA procedures rather than this general policy.
9. Do Not Track
Some browsers offer a “Do Not Track” signal. Because there is no industry-standard way to respond to these signals, our Site does not currently respond differently based on Do Not Track settings.
10. Links to Other Websites
The Site may contain links to third-party websites. We are not responsible for the privacy practices or content of those external sites. We encourage you to review the privacy policy of any site you visit.
11. Changes to This Privacy Policy
We may update this Privacy Policy from time to time. Changes will be posted on this page with an updated “Last Updated” date. Your continued use of the Site after changes are posted constitutes acceptance of the revised policy.
12. Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.
13. Embedded content from other websites
Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.
These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website.
14. Who we share your data with
If you request a password reset, your IP address will be included in the reset email.
15. What rights you have over your data
If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
