Enrollment

With the NEXTGEN® Patient Portal, you can connect with your doctor here at Salem Pediatric Clinic through a convenient, safe, and secure environment. Once you enroll, you can:

  • Exchange secure messaging with your doctors
  • Request medication renewals
  • Request appointments
  • Access your health record

In order to enroll, you may use the online enrollment system here, or you can request an 8-digit security token from Salem Pediatric Clinic and follow the steps below.

Do not use the Patient Portal for urgent medical needs. You will receive a response within 48 hours. Call our office at 503-362-2481.

Enrollment Steps

  • Step 1 — Fill out the request form

    Fill out the request form (and be sure to provide the email address you want linked to your Patient Portal account), then return it to SPC.

  • Step 2 — Request a security token from SPC

    You must request a NEXTGEN Patient Portal security token at the Salem Pediatric Clinic front desk.

  • Step 3 — Receive a security token from SPC

    Once the form has been returned to SPC, we will inform you of your security token in a welcome letter (usually within 5-7 days). NOTE: You will have 30 days to successfully enroll on the NEXTGEN Patient Portal site before the security token expires.

  • Step 4 — Go to the NEXTGEN Patient Portal to begin enrollment

    Once you have your security token, you may begin online enrollment at https://www.nextmd.com. Follow the steps, and enter your security token and email address to begin validation.

  • Step 5 — Receive validation of enrollment and create your account

    Once you have received your validated enrollment notification email from NEXTGEN, you will be able to create your Patient Portal account. NOTE: If you do not receive an enrollment notification email, please contact SPC.
